Safety

  • My Employee has been Injured, What Do I Do? (Supervisor Role)
    • Supervisor's Safety Report: This needs to be completed by the supervisor as soon as they receive the Employee's Safety Report or as soon as the injury or aggravation is reported to them, but no later than 24 hours afterward.
    • Contact Risk Management if you need a fillable First Report of Injury via WorkersCompensation@ci.stpaul.mn.us

School Year Hiring Paperwork

  • Supervisor to complete:
  • All New Youth to Complete:
  • Youth Application – this is so we can determine whether they are eligible.
  • Youth Participation Form - requirement of being hired
  • Workforce One Info Form – this information is required by our funders.
  • School Year Income Affidavit/Permission Slip – this information is also required by our funders.
  • Payroll Packet — required by every Right Track worker unless they had a YJ1 job in the summer.
  • All Returning Youth to complete:
    • Let us know if any of their contact info has changed (e.g. new email address, home address, phone number, etc).
