Overview

A coin-operated mechanical amusement device is any machine which, upon the insertion of a coin, token, or slug, operates or may be operated by the public for use as a game, entertainment, or amusement. It contains no automatic payoff device. This includes pinball machines, music machines, coin-operated television units, motion picture machines, and amusement rides (except amusement rides used at carnivals). Application Requirements

You must submit the following:

  • DSI Class R License Application
  • Listing of machine type (i.e., pool table, video game, pinball machine, rides, juke box) and their locations
  • Required fee for each machine

Expiration/Renewal 

A Mechanical Amusement Device License expires one year from date of issue and may be renewed annually.
