- Proof of employment hardship due to COVID-19:
- One or more documents in List A
- Proof of residency, income and minor child:
- One document from List B, or
- One document from List C AND all documents from List D
- Proof of residency in Saint Paul:
- Submit one from List E, only if the primary address on your Bridge Fund for Families application does not match the address on documents from Lists B, C or D.

LIST A
Submit one or more documents as proof of employment hardship due to COVID-19:
- Proof of enrollment in Unemployment Insurance, including receipt of Disaster Unemployment Assistance or Pandemic Unemployment Compensation
- If self-employment has been impacted, attach a letter outlining impact of COVID-19 on ability to generate income
- Letter or email from an employer documenting job loss or hours reduced due to COVID-19
- Evidence of unpaid temporary leave to self-quarantine, self-isolate, or to care for a dependent whose school or daycare has closed or who is considered at increased risk for complications from COVID-19

- Submit one or more documents from List B, or
- Submit one document from List C and all documents from List D

LIST B
Submit one or more documents as proof of residency, income, and minor child (under 18 as of April 19, 2020):
- 2019 Federal Income Tax Return – Form 1040, and if applicable, Schedule C for those with self-employment income)
- If you have not filed your 2019 Federal Return and your income did not significantly change between the time of filing and March 1, 2020, you may submit your 2018 Federal Income Tax Return
- Documentation that you are approved for FREE Meals through the Saint Paul Public Schools or another school participating in the National School Lunch Program
- Documentation that your child is enrolled in Head Start or Early Head Start
- Documentation that you receive Minnesota Family Investment Program (MFIP) benefits
- Documentation that you receive Child Care Assistance Program (CCAP) benefits

LIST C
Submit one document as proof of minor child (under 18 as of April 19, 2020)
Documentation only needs to be provided for one minor child (under 18 as of April 19, 2020) in the household, even if multiple minor children are present in the household.
- Birth certificate for child
- Social security card for child
- School ID or other government-issued ID for the child
- Evidence of receipt of child support
- Letter or email to parent/guardian from child’s school or daycare provider, such as a fee statement or report card
- Evidence of guardianship or status as a foster parent (e.g. copy of decree or court order)
- Proof of child’s enrollment in youth activity such as religious class, Saint Paul Parks and Recreation, youth sports, or Boys and Girls Club
- Medical insurance card or other medical insurance document showing proof of dependents, medical bill or immunization record

LIST D
Submit all documents as proof of income and residency
Documents must be provided for each household member over 18.
- Wages or Salary: 2019 W2s OR at least two consecutive paystubs dated between 1/1/2020 and 3/1/2020
- Self-Employment Income: 2019 1099-MISCs OR a profit and loss statement(s) covering the period between 1/1/2020 and 3/1/2020 OR documentation of revenue from business over one month between 1/1/2020 and 3/1/2020
- Other taxable income to include:
- Income from Rental Property
- Retirement Income or payments from Pensions, Annuities or IRA
- Disability Income (such as payment from insurance or workers compensation)

- Current lease agreement
- Mortgage statement dated between 1/1/2020 and 4/30/2020
- Photo of unexpired picture ID with the address listed as primary residence
- Bill, account statement, or start-of-service statement due or dated between 1/1/2020 and 4/30/2020:
- Phone, TV or internet
- Solid waste, sewer, electric, gas or water
- Banking or credit card
