LIABILITY WAIVER

RELEASE FROM LIABILITY AND COVENANT NOT TO SUE: The undersigned or his/her parent or guardian, if a minor agrees to release the City of Saint Paul, its employees, officers, directors, or agents from any and all liability from claims of any nature whatsoever arising from undersigned’s participation in Blooming Saint Paul Day on May 18th, 2013, except to the extent such liability is directly attributable to the gross negligence or willful or wanton conduct of the City, or its employees, officers, directors or agents.

PARTICIPANT ASSUMES RISK: The undersigned (or his/her parent or guardian, if the undersigned is a minor) understands the inherent risks and dangers of participation in a physical activity, and agrees to assume all risk of and responsibility for personal injury or death, or damage to property arising from, based upon, or relating to the participation in the event.

PHOTO WAIVER

RIGHT OF PUBLICITY: Participation in the Saint Paul Spring Parks Cleanup shall constitute permission to use the name, likeness or any other identification of the Participant in City publications and its web page without compensation to or right of prior review or approval by the Participant or his/her parent or guardian (except where prohibited by law).
